Brief introduction

Somarlutide, developed by Novo Nordisk and marketed under the Ozempic, Wegovy and Rybelsus brand names, is an anti-diabetic drug for the treatment of type 2 diabetes and an anti-obesity drug for long-term weight management.

Peptides can be administered by subcutaneous injection or orally.

In 2020, it was the 129th most commonly used drug in the United States, with more than 4 million prescriptions written.

Indications and uses

Somarlutide can be used as an adjunctive to diet and exercise to improve blood sugar control in adults with type 2 diabetes and to increase physical activity for chronic weight loss

The initial body mass index (BMI) is the management of the following adults:

30 kg/m2 or above (obese) or

27 kg/m2 or higher (overweight) with at least one weight-related comorbiditis (such as hypertension, type 2 diabetes, or dyslipidemia).

Action mechanism

Semaglutide is a GLP-1 analogue that shares 94% sequence homology with human GLP-1. Sommarutide acts as a GLP-1 receptor agonist, selectively binding to and activating the GLP-1 receptor, the target of natural GLP-1.

GLP-1 is a physiological hormone that is mediated by GLP-1 receptors and has a variety of effects on glucose.

The main mechanism leading to the long half-life of sommarutide is albumin binding, which leads to reduced renal clearance and prevents metabolic degradation. In addition, sommarutide is stable and prevents degradation of DPP-4 enzymes.

Sommarutide lowers blood sugar through mechanisms that stimulate insulin secretion and decrease glucagon secretion, both in a glucose-dependent manner. Therefore, when blood sugar is high, insulin secretion is stimulated and glucagon secretion is suppressed. The mechanism of lowering blood sugar also involves a slight delay in gastric emptying early after a meal.
